Optimization of the treatment of chronic diseases.
In our practice, we offer Disease Management Programs (DMPs) for patients with chronic diseases such as diabetes, COPD/asthma, and coronary heart disease (CHD). This structured treatment program is a proven method to facilitate dealing with these diseases and to improve the quality of life.
Since 2002, DMPs have been offered in Germany and include regular doctor's appointments with counseling sessions and check-ups. The aim of DMPs is to improve collaboration between different professionals and facilities and to coordinate treatment steps with each other.
As a participant in the DMP, you will receive individual care from our doctors and our medical staff. We work together with you to improve the treatment of your chronic disease and to increase your quality of life. Through close monitoring, possible complications or sequelae can be detected and avoided at an early stage.

How can I participate in the Disease Management Program?
If you suffer from diabetes, COPD/asthma or CHD, you are welcome to contact us in our practice. To participate in the DMP program, you need a corresponding diagnosis and must be insured with a statutory health insurance company. Participation in our DMP program is completely free for you.
Please bring any existing medical reports with you to your appointment in our practice. We will then discuss together with you to what extent participation in the DMP program makes sense for you and what steps are necessary for this.
